Before an appointment can be scheduled, the required laboratory requisition, kit draw form, and/or physician order must be completed.

Please also provide a copy of the patient's insurance card, front and back, when applicable.

Important: Requisition forms and physician orders must be completed and submitted before an appointment can be scheduled. This allows our phlebotomist to arrive prepared with the appropriate equipment and properly process the specimen according to the laboratory's requirements.

Please follow your healthcare provider's instructions before your blood draw or specimen collection. This may include fasting or drinking plenty of water before your appointment.

Our trained staff will arrive at the approved location and collect the requested specimens using appropriate collection procedures and equipment.

After collection, specimens are handled and transported according to the applicable laboratory requirements and protocols.

For patient testing, results are sent directly to the ordering healthcare provider.

For research cases, ProLab Solutions utilizes electronic tracking to help ensure specimens are properly identified and delivered according to the required protocol.
